Management of high risk patients

Abstract

All countries, rich and poor, struggle with how to improve the health of the population and obtain the most value from medical services, and restrain rising health care expenditures (Schoroeder, 2007; Seligman & Schillinger, 2010). Many developed nations also wrestle with the paradox that their citizens have never been so healthy or so unhappy with their medical care (Schoroeder, 2007; Seligman & Schillinger, 2010). Thus, in an attempt to diagnose and promptly to treat the main risk factors for prevalent diseases, we developed a sophisticated program that can automatically identify and monitor patients with these characteristics and place them in treatment and or prevention programs in an attempt to achieve better medical outcomes and cost-effectiveness.
