Budget impact analysis of the introduction of HPV vaccine in the Brazilian Public Healthcare System: a tool for policymakers

Abstract

Objective: To estimate the budget impact of the introduction of the HPV vaccine in the Brazilian Public Healthcare System. Methods: To estimate the budget impact, the study uses a scenario based on scientific evidence that considers previously expected vaccine coverage and respects thirteen fundamental assumptions. The study creates different subgroups of analysis according to different age groups. To calculate the cost of the vaccine we add the vaccination cost to the logistic cost. Results: Vaccination is the most expensive in the first year, i.e. in 2013, if considered more than one age. However, in the following year, a drop occurs on the budge impact and in the last year there is a marginal increase on the impact. The results indicate that vaccination of the subgroup comprising all girls between ages of 9 and 13 is unfeasible since the costs amount to almost 40% of the budget of the National Program on Immunization. The budgetary impact of the tetravalent vaccine is equivalent to the bivalent vaccine in terms of trajectory. However, in general, the budget impact of this vaccine is slightly larger than that of the bivalent, as the tetravalent is slightly more expensive. Conclusion: When vaccination covers two age groups, the incorporation of HPV vaccine in the Brazilian Public Healthcare System seems reasonable.
